Palliative & End-of-Life Care

We believe everyone has the right to die with dignity in the place of their choosing, surrounded by those they love. By enrolling in our Palliative Care Service we will provide you with a personalized medical care plan based on your specific needs. We will provide onsite medical care and day or night everyday of the week.  Our commitment is to you and your loved one.

 Services include:

• Palliative Assessment & Personalized Care Plans with 48 hours

• Access to Nurse Practitioner and/or Medical Physician (24 hours a day, 7 days a week)

• Nurse Practitioner & Registered Nurse specializing in Palliation

• Regular in-home visits from the medical team

• Complex and acute pain and symptom management

• Support with establishing goals of care, goal setting, and advanced care planning

• Psychological support to patients, caregivers and families

• Care and comfort at the end of life 

• Support with physical, spiritual, emotional, social and informational needs

• Support for patients and families with difficult end of life decisions(i.e withdrawal of treatment)

• Support transition from inpatient care facility back to home

• Assistance with paperwork, documents and forms

• MAID (Medical Assistance in Dying)

• Pronouncement of Death (completion of death certificate)
